Please provide two of the following. Visit my FAQ page to learn about how I protect your information and privacy.
If uploading ID, feel free to redact your home address & identifying numbers (DLN, DD, passport #, etc.) leaving everything else clear & visible.
- Employment info: enough to sufficiently verify your professional identity. LinkedIn, link to corporate profile, etc.
- References: name, email and website for two companions you’ve seen within the last 12 months.
- An upload of your government ID.
- A photo of you holding your government ID next to your face.
Email or Signal Submission
If you’d rather bypass my form, simply email me and let me know you’re ready to book. I’ll reply with the information I need from you to complete screening. alyxchi@proton.me.
And if you’d like to send your screening information through Signal, simply mention that in your email and I’ll provide my Signal number.
